Qualifications

• Bachelor's degree in an appropriate field or an equivalent combination of education and experience.
• 1-year experience with HIV population, adult/pediatric outpatient clinic, NCQA PCMH and Quality Improvement Projects.

Responsibilities

• Coordinate requirements and logistics for the PCMH annual recognition.
• Coordinate patient services and administrative activities of clinical data; monitor NCQA regulations.
• Develop and coordinate patient services to review NCQA standards; assist Quality Officer with COI and strategic plans.
• Monitor patient care plans and treatment problems; collaborate with clinical team members.
• Maintain records and reports related to NCQA/PCMH; track data in Excel.
• Coordinate PCMH committee meetings and participate in health-related task forces and advisory committees.
• Attend NCQA workshops and webinars.
• Provide health education to patients, staff, and practitioners related to NCQA compliance.
