Qualifications

• Bachelor's degree or equivalent combination of education and experience.
• 1 year experience with NCQA PCMH and quality improvement projects (preferred).

Responsibilities

• Develops and coordinates patient services to review NCQA standards.
• Assist and support the Quality Officer to review and monitor NCQA/PCMH plans.
• Assist providers by monitoring patient care plans and treatment issues.
• Collaborate with the clinical team on patient treatment plans.
• Maintain NCQA/PCMH records and track data in Excel.
• Coordinate PCMH committee meetings and participate in health task forces (CEG).
• Attend NCQA workshops and webinars.
• Provide administrative support for additional needs.
• Provide health education to patients, staff, and practitioners related to NCQA compliance.
