Job summary

Crossroads Inc is seeking a Clinical Coordinator to support outpatient and residential programs under the Outpatient Programs Administrator. The role includes supervising the clinical team, conducting assessments, developing and updating treatment plans, and coordinating care with aftercare services.

Qualifications include a high school diploma or GED (Bachelor’s degree preferred) and relevant behavioral health experience.

Responsibilities

• Oversee clinical team, assign caseloads, schedule staff and audit records.
• Perform client assessments to determine needs and treatment goals.
• Develop and monitor individual treatment plans and progress notes.
• Prepare intake documents and ensure regulatory compliance.
• Monitor staff caseloads and documentation timeliness.
• Provide individual and group interventions including crisis management.
• Coordinate aftercare and facilitate case reviews and trainings.
• Participate in staff meetings and contribute to training.
• Perform other duties as required.
