Job summary

Advocate Health in Park Ridge, IL, is seeking a licensed Physical Therapist to evaluate and treat patients across musculoskeletal, neuromuscular, cardiopulmonary and integumentary systems. You will establish plans of care, implement treatments, monitor outcomes, and coordinate with physicians and teammates to ensure optimal patient progress.

You will complete documentation, discharge planning, and patient education, while maintaining productivity standards and adhering to safety and infection

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state where you work; BLS for Healthcare Providers required.
• No experience required.
• Strong communication, interpersonal, and organizational skills.
• Ability to meet required physical demands and travel if needed.

Responsibilities

• Interpret referrals and perform musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations.
• Develop and implement individualized treatment plans with ongoing outcomes evaluation.
• Discharge planning and patient/family teaching; refer for community services.
• Complete documentation—evaluations, progress notes, discharge summaries, and billing.
• Maintain productivity standards and ensure safety and infection control.
