Job summary

Recover-Care is seeking a licensed Physical Therapist to develop individualized therapy programs for SNF patients, including assessment, treatment planning, and progress communication with the care team.

You will perform evaluations, design plans of care, document goals, and participate in ongoing professional development while adhering to standards and ensuring patient safety and quality outcomes.

Qualifications

• Graduate of an accredited PT program (Bachelors/Masters/Doctorate).
• Licensed to practice physical therapy in the state.
• Knowledge of SNF regulatory standards.

Responsibilities

• Provides direct patient care, evaluates functional needs and outcomes, and adjusts care plans accordingly.
• Designs and implements a plan of care based on comprehensive assessment and stakeholder involvement.
• Conducts therapy within defined standards of care.
• Directs the assistance of support staff to achieve patients' functional goals.
• Communicates effectively with other professionals and staff to optimize patient outcomes.
• Follows facility policies, safety, and infection control standards.
• Documents evaluations, treatment goals, and plans; maintains records and signatures.
• Pursues ongoing professional development through courses, in-service trainings and literature review.
