Responsibilities

• Has knowledge of therapeutic principles and practices with good clinical judgment. Maximizes patient safety and treatment potential.
• Identifies and records problems areas on evaluation; projects goals with realistic time frames.
• Analyzes and designs treatment plans based on patient problems and goals; assesses progress.
• Prepares for patient discharge with organized, efficient submission of equipment needs.
• Fosters teamwork by fair treatment, open communication and collaboration.
• Organizes patient treatment time efficiently.
• Records patient progress concisely and timely.
• Performs other duties as assigned.
