Cantex Continuing Care Network is seeking a Physical Therapist to provide comprehensive PT services, conduct evaluations, and develop individualized treatment plans to improve mobility and independence. You will collaborate with physicians and families while ensuring regulatory compliance.

The role includes supervising PT Assistants, documenting progress, and educating patients and caregivers on recovery goals. A $6K sign-on bonus is offered.

Formación

• Graduate of an accredited School of Physical Therapy.
• Current licensure as a Physical Therapist in the applicable state.
• Strong English communication skills, verbal and written.
• Ability to assess patient needs and implement effective interventions.
• Ability to meet health, physical, and regulatory requirements.
• Ability to work with patients, families, and healthcare teams.

Responsabilidades

• Screen, evaluate, and treat patients requiring physical therapy services.
• Complete comprehensive patient evaluations utilizing appropriate assessment tools.
• Review medical records and radiology, if applicable, to support planning.
• Establish measurable goals and develop individualized treatment plans.
• Supervise Physical Therapist Assistants and Rehabilitation Technicians where allowed.
• Communicate evaluation findings and progress with patients, families, and physicians.
• Collaborate with healthcare teams to implement and adjust treatment plans.
• Maintain accurate patient documentation and billing-related records.
• Participate in care conferences and interdisciplinary coordination.
• Educate patients and caregivers on maintenance programs and goals.
