Job summary

Cantex Continuing Care Network is seeking a licensed Occupational Therapist to evaluate and treat patients with physical, developmental, cognitive, or functional limitations. You will develop personalized treatment plans to improve daily living activities, independence, and overall quality of life, while coordinating with physicians and care teams.

Responsibilities include timely documentation, care plan updates, discharge planning, and educating families about maintenance programs.

Qualifications

• Ability to communicate effectively in English, both verbally and in writing.
• Strong problem-solving skills necessary to assess patient needs and implement effective therapeutic interventions.
• Strong communication and interpersonal skills with the ability to work effectively with patients, family members, facility staff, and interdisciplinary healthcare teams.

Responsibilities

• Screen, evaluate, and treat patients requiring occupational therapy services.
• Complete comprehensive occupational therapy evaluations utilizing appropriate assessment tools and clinical procedures.
• Review patient records, diagnoses, medical history, precautions, and contraindications to support treatment planning.
• Establish measurable goals and develop individualized treatment plans that maximize patient independence and functional performance.
• Supervise Occupational Therapy Assistants and Rehabilitation Technicians in accordance with state practice regulations.
• Communicate evaluation findings, treatment plans, and patient progress to patients, families, physicians, and interdisciplinary team members.
• Collaborate with physicians and healthcare teams to implement, monitor, and revise plans of care based on patient progress and clinical needs.
• Maintain accurate and timely documentation including evaluations, treatment records, care plan updates, discharge summaries, and billing-related records.
• Participate in patient care conferences, rehabilitation meetings, care coordination activities, and discharge planning efforts.
• Educate caregivers and family members regarding patient-specific maintenance programs and therapeutic techniques.
• Serve as a clinical preceptor for occupational therapy educational programs as assigned.
• Ensure compliance with occupational therapy practice standards, regulatory requirements, productivity expectations, and patient safety initiatives.
