Job summary

The Jewish Hospital (Bon Secours Mercy Health) seeks a Clinical Pharmacy Specialist to deliver patient-centered pharmaceutical care across hospital departments in Cincinnati. You will coordinate care with nurses and physicians, monitor therapies, and participate in programs to ensure safe, effective, and cost-conscious pharmacotherapy.

Required are a PharmD/BS with PharmD training and licensure in the relevant state(s). Hospital experience and EPIC proficiency are highly valued.

Qualifications

• Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD (required)
• Advanced pharmacy degree (PharmD, Residency) preferred
• Maintains current pharmacist license in state(s) of practice (required)

Responsibilities

• Conduct initial and ongoing patient assessment with the care team
• Administer medications safely per policy and state regulations
• Serve as point of contact for patient coordination across hospital departments
• Participate in quality improvement and safety initiatives
• Provide comprehensive pharmaceutical care and drug monitoring
• Support therapeutics and educational activities for patients and staff
