Qualifications

• Licensed in the state of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification required.
• Credentialing and privileging required.
• Bachelor’s or Doctorate in pharmacy or FPGEC certificate.

Responsibilities

• Oversees pharmaceutical care by evaluating literature and designing therapy regimens.
• Counsels patients and caregivers on therapy, adverse effects, compliance, and administration.
• Designs and monitors patient-specific regimens based on literature; implements and documents.
• Reviews medication orders and participates in medical emergencies when applicable.
• Supports medication safety, quality improvement, and formulary management.
• Precepts pharmacy students and mentors teammates.
