Vytalize is seeking a Clinical Pharmacist to lead pharmacist-driven clinical programs, engage providers and patients, and optimize medications within a value-based care framework.

You will collaborate with medical leadership and care teams to deliver MTM, CMR, and medication reconciliation, while supporting drug strategy and decision support enhancements to improve outcomes and total cost of care.

Qualifikationen

• PharmD required and licensed to practice in the U.S.
• Experience with Medication Therapy Management (MTM) and pharmacist-led clinical programs.
• Bilingual Spanish speaking preferred/required for patient engagement.

Aufgaben

• Lead pharmacist-driven clinical programs and engage providers and patients.
• Perform MTM, CMRs, and medication reconciliation to optimize therapy.
• Provide telephonic or virtual patient education and action-oriented recommendations.
• Collaborate with care teams to implement clinical decision support and drive program adoption.
• Support compliance, documentation, and quality initiatives across markets.
