Vanderbilt Health, based at Vanderbilt University Hospital in Nashville, seeks a Clinical Pharmacist to lead patient-centered pharmacotherapy and ensure safe, cost-effective medication use across the care team.

The role emphasizes daily patient review, transitions of care, antimicrobial stewardship, and education for providers, nurses, and patients, with evening shifts and potential relocation assistance.

Formación

• ASHP-accredited PGY1 residency required or equivalent experience.
• Experience in clinical practice setting.
• Board of Pharmacy Specialties (BPS) certification in pharmacotherapy or related specialty.

Responsabilidades

• Reviews drug orders and prescriptions to ensure safe, appropriate and cost-effective use of medications.
• Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
• Maintains the quality, safety, and consistency of clinical pharmacy services.
• Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prior to dispensing.
