Job summary

Vanderbilt Health is seeking a Clinical Pharmacist to lead a specialized team, ensuring optimal, cost-effective drug therapy for adult medicine patients. The role includes developing pharmacotherapeutic goals, monitoring plans, and leading efforts to prevent medication errors and adverse drug reactions within a dynamic, teaching and non-teaching environment.

Qualifications

• ASHP-accredited PGY2 residency or other post-graduate training in internal medicine.
• ASHP-accredited PGY1 residency.
• Equivalent years of experience in internal medicine or clinical practice.
• Board of Pharmacy Specialties (BPS) certification in pharmacotherapy or other specialty certification.

Responsibilities

• Reviews drug orders and prescriptions to ensure safe, appropriate and cost-effective use of medications.
• Prepares and dispenses medications and provides drug information.
• Monitors and optimizes medication therapy for patients.
• Supervises support personnel and verifies accuracy of medications before dispensing.
• Participates in interdisciplinary rounds and manages transitions of care.
