Qualifications

Description

Summary:


All pharmacists fill orders for drugs, monitor patient drug therapies, and provide drug information. Pharmacists deliver pharmacy services with compassion, excellence, and efficiency.


Responsibilities:


  • Meet expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
  • Prepare and dispense drug orders per physician request according to established policies, procedures, and protocols.
  • Interpret drug orders (verbal and written) and transcribe/verify into computerized patient medication records accurately. Maintain accurate and complete patient drug records.
  • Compound and dispense pharmaceuticals, including non-sterile and sterile products, chemotherapy, and parenteral nutrition products accurately.
  • Issue controlled substances to patient care areas and maintain records as required by law and institutional policies.
  • Provide services efficiently and in a timely manner.
  • Maintain current pharmacist license, ensure applicable CE records and licensure are up-to-date, and attend staff meetings.
  • Complete all competency and skills assessment requirements.
  • Supervise and direct pharmacy support personnel; verify daily activities of pharmacy technicians.
  • For ambulatory care pharmacists: provide pharmaceutical care services, manage patient drug therapy, anticoagulation, patient education, and oversee medication preparation and dispensing in clinical areas such as anticoagulation clinics, transition of care, discharge medication reconciliation, etc.
  • Ensure safe, appropriate, and cost-effective drug therapies according to policies and protocols.
  • Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness of drug and dose.
  • Assist with pharmacokinetics consultations and renal drug dosing per hospital protocol.
  • Read, extract, and interpret information from patient medical records accurately.
  • Detect and report suspected adverse drug reactions promptly.
  • Sustain the hospital drug formulary, minimize non-formulary procurements, utilize therapeutic substitution protocols, and promote rational drug therapy.
  • Provide clinical consultation and clarification to practitioners as needed.
  • Provide accurate and timely drug information to hospital staff.
  • Educate patients and families about medications per institutional protocols.
  • Participate in quality improvement and medication review activities, including data collection, monitoring, and record keeping.
  • Conduct designated interventions such as IV to PO conversions, renal dosing, pharmacokinetic support, and anticoagulation management, as defined in departmental standards.
  • Participate in antibiotic stewardship, opioid stewardship, and shortage management activities.
  • Attend interdisciplinary rounds when assigned.
  • Potential exposure to hazardous substances, biologic material, or sharps; follow safety protocols to minimize risk.

Requirements:


  • Doctor of Pharmacy or BS Pharmacy degree required.
  • Hospital experience preferred.
  • Pharmacy registration in the state of practice.
  • Pharmacy licensure within 60 days of start date.
  • BLS or ACLS and PALS (pediatric practice) certifications within 180 days of start date.
