Description
JOB SUMMARY: The Clinical Pharmacist I optimizes patient care by providing appropriate, safe and cost-effective pharmaceutical care including drug dispensing, IV admixture program, and providing drug information. The Clinical Pharmacist I works under the direction of the Director of Pharmacy and/or the Pharmacy Operations Manager and will oversee the work of pharmacy technicians, students, and support staff.
MAJOR RESPONSIBILITIES / DUTIES / CRITICAL TASKS:
- Functions in accordance with standard written policy & procedures, guidelines, and state and federal regulations.
- Communicates with other pharmacy team members and caregivers effectively. Demonstrates emotional control and intelligence in managing work relationships. Attends and actively participates in team huddles and meetings.
- Answers phones professionally and pleasantly.
- Interprets orders and transcribes or verifies them accurately and efficiently in the pharmacy computer system.
- Accurately compounds and dispenses pharmaceuticals.
- Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
- Provides accurate and timely medication information to healthcare staff and patients.
- Effectively interacts with medical staff regarding drug therapy issues.
- Documents all clinical activities and interventions accurately and completely.
- Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and other approved protocols.
- Supervises and assists pharmacy technicians and support staff effectively.
- Trains and participates in Disaster Team drills and real-event response as assigned.
- Prioritizes work and leads others to comply with medication turnaround time policies and identify areas for efficiency improvements.
- Participates in staff meetings and pharmaceutical care development activities.
- Supports training and mentorship of new staff; may serve as preceptor for students/residents.
- Supports quality improvement, risk management, and patient safety programs, including national safety goals and initiatives.
- Maintains professional competency and appropriate controlled substance records.
- Ensures compliance with all regulatory standards.
- Performs other duties as assigned.

Required Education / Experience:
- B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.
- Pharm D. preferred.
- 1 year inpatient hospital pharmacy experience preferred.
Required Certifications / Licenses:
- Active Pharmacist license in good standing with the State Board of Pharmacy.
- Additional state requirements (e.g., sterile compounding).
- Current BLS and ACLS certifications.
