Qualifications
- Pharmacist must hold a PharmD or a Bachelor’s in Pharmacy.
- 0–1 year hospital or retail pharmacy experience required.
- Licensed pharmacist in the state of employment (RPH).
Responsibilities
- Accurately prepares, verifies, and dispenses medications according to orders and protocols.
- Reviews orders, assesses regimens, and recommends therapy optimizations.
- Counsels patients and families on proper use and adherence.
- Collaborates with physicians, nurses, and healthcare team to improve outcomes.
- Participates in medication safety, quality improvement, and audits.
- Maintains documentation of interventions in the EHR and ensures regulatory compliance.
- Stays current with pharmacotherapy advances and ongoing education.
- Manages pharmacy inventory and identifies cost-saving opportunities.
