Job summary

Valley Health seeks a pharmacist to ensure formulary and interchange protocol compliance, educate staff, and improve medication management across patient care settings. The role emphasizes safety, documentation, and effective collaboration with prescribers and nursing staff.

Responsibilities include leading safety initiatives, managing transitions of care, and training staff while maintaining licensure requirements and engaging in continuous professional development.

Qualifications

• BS in Pharmacy or Pharm D required.
• Virginia or West Virginia Pharmacy License required (or acquired within 90 days of hire).
• Knowledgeable about medications, pharmacology and dosage ranges across patient ages.

Responsibilities

• Ensure formulary/interchange compliance and educate staff.
• Initiate and monitor medication safety processes and report ADEs.
• Perform patient medication management and transitions of care.
• Coordinate practice management and formulary submissions to P&T.
• Direct technician shifts and oversee workflow and accuracy.
• Educate staff and serve as a preceptor for new personnel.
• Maintain licensure compliance and participate in ongoing education.
