Qualifications

Responsibilities

• Accurately prepares, verifies, and dispenses medications in compliance with prescription orders and hospital protocols
• Reviews medication orders for appropriateness and optimizes therapy
• Counsels patients and families on medication use, side effects, and adherence
• Collaborates with physicians, nurses and other health-care team members to resolve medication-related issues
• Participates in medication safety initiatives, quality improvement projects and audits
• Maintains up-to-date documentation of interventions and patient consultations in the EHR
• Monitors outcomes and adjusts therapy to improve safety and efficacy
• Assists in inventory management and cost-saving opportunities
