Qualifications

• Pharmacist license active in the state of employment.
• 0–1 year of hospital or retail pharmacy experience.

Responsibilities

• Safely prepares, verifies, and dispenses medications per orders and regulations.
• Review orders, assess regimens, and optimize therapy for patients.
• Counsel patients and families on medication use and adherence.
• Collaborate with physicians and nurses to resolve medication issues.
• Participate in safety, quality, and compliance initiatives.
• Develop and implement clinical protocols to standardize use.
• Monitor outcomes and adjust therapy as needed for safety and efficacy.
• Document interventions and counseling in the electronic health record.
• Assist with inventory management and cost-saving opportunities.
• Engage in ongoing education and professional development.
