Qualifications

Responsibilities

• Review and verify medication orders with clinical interventions.
• Complete pharmacy consults per policies and procedures.
• Assess patient health problems and ensure optimal medication therapy.
• Lead or participate in hospital pharmacy committees as assigned.
• Precept, educate, and train pharmacy personnel, students, and residents.
• Engage in patient care planning within a multidisciplinary team.
