Qualifications

• BS in Pharmacy required.
• PharmD degree required.
• ASHP-accredited residency plus 1 year hospital/clinical experience.
• Completed equivalent clinical fellowship.
• 3 years relative healthcare system experience.

Responsibilities

• Review, interpret, and verify provider medication orders with clinical input.
• Complete pharmacy consults following policies and procedures.
• Assess patient health problems and ensure medications meet goals of care.
• Identify untreated health problems treatable by medication therapy.
• Monitor patient progress and medication effects.
• Participate in hospital/pharmacy committees as assigned.
• Precept, educate, and train pharmacy staff, students, and residents.
• Engage in multidisciplinary patient care planning at least 50% of schedules.
• Respond to emergent calls and ensure safe medication use.
• Educate patients on medication use and preventive steps.
