Responsibilities

• Check all medications leaving the department for accuracy and quality.
• Possess working knowledge of chemotherapy agents, hyperalimentation, IV admixture, narcotic control, investigational drugs, and therapeutic drug monitoring.
• Ensure correct entry of orders into the computer and provide current medication administration reports to hospital personnel.
• Verify dosages on physician orders for patient appropriateness and ensure timely dispensing of medications.
• Communicate with healthcare professionals regarding orders, drug interactions, allergies, and dosing questions.
• Collaborate with staff to plan new functions and update pharmacy services to maintain high standards.
• Participate in in-service programs and department projects as assigned.
• Competent in using computer systems, chart writing, and dosing based on patient parameters.
• Enter oral and IV medications into HBOC System; oversee Omnicell System; dose drugs in PKD Service; monitor adverse drug reactions.
