Job summary

Job description

Description

The clinical pharmacist plays a pivotal role in delivering high‑quality, individualized, cost‑effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient medication therapy outcomes and quality of life. All duties are performed in a manner that promotes teamwork and reflects the Sisters of Providence mission and philosophy.

Key Responsibilities
  1. Provide comprehensive pharmaceutical patient care.
  2. Follow department, facility, and regional policies, procedures, and guidelines related to medication use, pharmaceutical care, and documentation of clinical interventions.
  3. Communicate effectively both verbally and in writing with key stakeholders such as peers, caregivers, patients, and providers.
  4. Maintain knowledge of regulatory requirements and medication safety standards.
Required Qualifications
  • PharmD Doctorate of Pharmacy or Bachelor’s Degree in Pharmacy.
  • Washington Pharmacist License upon hire.
  • Basic Life Support (BLS) certification where applicable.
  • Board of Pharmacy Preceptor License when eligible and where applicable.
Preferred Qualifications
  • Acute‑care hospital pharmacy experience, 1 year or equivalent.
  • Completion of an ASHP‑accredited PGY1 residency.
