Job summary

Advocate Health in Chicago seeks a pharmacist to oversee pharmaceutical care, design therapy regimens, and educate patients and caregivers. You will interpret orders, ensure safety, and collaborate on formulary management while mentoring learners and staff.

The role requires a Bachelor’s or Doctorate in Pharmacy, licensure in the appropriate state, and immunization certification; life support training may be required depending on department needs. This is a full-time hospital position.

Qualifications

• Licensed pharmacist in the states of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification required depending on practice area.
• Credentialing and privileging required based on area of practice.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens.
• Counsels patients and caregivers on therapy, adverse effects, compliance and handling.
• Interprets medication orders and prescriptions accurately.
• Supports medication safety and quality improvement activities.
• Collaborates with healthcare team on formulary adherence and procurement.
• Prepares, dispenses, and ensures proper storage of medications per regulations.
• Serves as preceptor for students and residents; mentors staff as assigned.
• Complies with all policies, procedures, and regulatory requirements.
