Job summary

Advocate Health is seeking a Pharmacist at the Illinois Masonic Medical Center Pharmacy in Chicago to provide comprehensive pharmaceutical care and educate patients and caregivers on therapy and safety.

You will collaborate with the health care team to ensure formulary adherence, accurate dispensing, and compliant operations in a hospital setting, with hospital experience preferred and licensure required.

Responsibilities

• Provides patient counseling on medication therapy, adverse effects, compliance, handling, and administration.
• Designs, monitors, and evaluates pharmacotherapeutic regimens based on biomedical literature and policies.
• Reviews and interprets medication orders and prescriptions accurately.
• Participates in medical emergencies when applicable.
• Supports medication safety and quality improvement activities.
