Job summary

UofL Health is seeking a Clinical Pharmacist to provide specialized pharmaceutical services, participate in rounds, evaluate orders, supervise technicians, and deliver patient counseling to improve outcomes. They may help develop new programs and professional activities.

You will collaborate with pharmacy leadership to ensure safe, economical pharmacotherapy and support clinical services across neonatal to geriatric patients, working with physicians, nurses, and other health professionals.

Qualifications

• PharmD degree from an ACPE-accredited college.
• Licensed or eligible for Kentucky pharmacy license.
• Board certification by BPS (or within 2 years).
• PGY-1 Pharmacy Residency or 3 years of relevant work experience.

Responsibilities

• Delivers comprehensive pharmaceutical care to patients.
• Participates in multidisciplinary patient care rounds.
• Reviews medication orders for accuracy, appropriateness and efficacy.
• Documents all pharmacy interventions in patient EMR and decision support system.
• Monitors drug therapy for appropriateness, dosing, and interactions.
• Performs clinical pharmacology consultations and dosing recommendations as required.
• Prevents adverse drug reactions and medication errors.
• Collaborates with leadership to implement clinical and operational initiatives.
