Job summary

Qualifications

• Licensed pharmacist with NH license and strong clinical pharmacy background.

• Ability to work within multidisciplinary teams and clinical guidelines.

Responsibilities

• Provide safe medication therapy and verify dosing, interactions, and contraindications.
• Conduct daily clinical chart reviews and intervene as needed.
• Counsel patients at discharge on new or modified medications.
• Supervise pharmacy technicians and serve as a preceptor for students.
• Participate in performance improvement and CQI activities.
