Job summary

St. Joseph's Health in Wayne, NJ seeks a pharmacist to perform clinical interventions, education, and counseling related to medication management and safety.

The role requires evaluating orders, preventing adverse interactions, and ensuring safe, accurate dispensing in collaboration with the care team. You will provide direct patient care across diverse ages, maintain ACLS readiness, and participate in ongoing professional development with CEU requirements.

Qualifications

• PharmD required; NJ license and registration.
• Board Certification in clinical pharmacy preferred (e.g., BCPS).
• ACLS within 1 year of hire.
• Direct patient care across neonatal to geriatric populations.
• 25 CEUs annually as approved by Pharmacy Management.
• Able to staff a minimum of 4 shifts per month on weekends/ weekdays.

Responsibilities

• Perform and document patient pharmacy clinical interventions, education, and counseling on medication management and safety.
• Interpret, evaluate, and review physician orders and outpatient prescriptions to prepare and dispense appropriate medications.
• Assess for drug allergies, interactions, duplications, dosing, and inappropriate therapy; resolve deficiencies.
• Ensure accurate dispensation of medications and collaborate with other health care practitioners to promote patient safety and optimal care.
