Job summary

OSS Health in York, Pennsylvania seeks a PRN Pharmacist to support both inpatient and outpatient care. The role includes medication management, clinical reviews, and collaboration with physicians to optimize patient outcomes.

Qualified candidates hold PharmD or equivalent pharmacist license with PA licensure and PGY1 or equivalent experience. PRN coverage aligns with FT staff PTO and includes varied shifts, with on-site work at OSS Health facilities.

Qualifications

• Doctor of Pharmacy (PharmD) or Registered Pharmacist license from an accredited college or school of pharmacy.
• Active pharmacist license in the State of Pennsylvania.
• Completion of a PGY1 pharmacy residency or equivalent clinical experience.
• Strong knowledge of pharmacotherapy principles, medication safety, and clinical practice standards.

Responsibilities

• Consult with medical staff regarding potential drug interactions, patient medical history, and use of particular medications.
• Collaborate with medical staff to plan, monitor, review, and evaluate the quality safety and effectiveness of medication regimens.
• Analyze prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions.
• Assess the identity, strength, or purity of medications; compound and dispense as prescribed by physicians and other providers.
• Provide direct clinical pharmacy services to patients, including medication therapy review, optimization, and monitoring.
• Support antimicrobial prophylaxis, VTE prophylaxis, analgesia, and anesthesia-related pharmacotherapy.
