Job summary

OSS Health seeks a pharmacist to provide direct clinical services, medication management, and safety oversight across inpatient, outpatient, and surgical settings. You will collaborate with medical staff to optimize regimens, monitor drug interactions, and ensure regulatory compliance.

The role requires PharmD or active pharmacist licensure, residency or equivalent experience, and strong knowledge of pharmacotherapy and medication safety.

Qualifications

  • Doctor of Pharmacy (PharmD) or Registered Pharmacist license from an accredited college or school of pharmacy
  • Active pharmacist license in the State of Pennsylvania
  • Completion of a PGY1 pharmacy residency or equivalent clinical experience
  • Strong knowledge of pharmacotherapy principles, medication safety, and clinical practice standards
  • Ability to work effectively in inpatient, outpatient, and surgical care environments
  • Experience with electronic health records and clinical decision support systems
  • Excellent interpersonal and customer service skills
  • Strong attention to detail, organizational skills, and ability to solve problems
  • Ability to balance competing priorities
  • Commitment to patient-centered care and continuous quality improvement
  • Knowledge of regulatory and accreditation requirements related to medication use

Responsibilities

  • Consult with medical staff regarding potential drug interactions, patient medical history, and use of particular medications
  • Collaborate with medical staff to plan, monitor, review, and evaluate the quality safety and effectiveness of medication regimens
  • Analyze prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions
  • Assess the identity, strength, or purity of medications; compound and dispense as prescribed or oversee these activities
  • Ensure pharmacy data is recorded, maintained, and complies with regulations, security, and OSS Health procedures
  • Provide direct clinical pharmacy services to patients, including medication therapy review, optimization, and monitoring
  • Participate in medication management, including antimicrobial prophylaxis, VTE prophylaxis, analgesia, and anesthesia-related pharmacotherapy
  • Support evidence-based pain management strategies, including multimodal analgesia, opioid stewardship, and management of acute and chronic pain
  • Review, verify, and optimize medication orders for appropriateness, safety, dosing, and therapeutic efficacy
  • Identify, prevent, and manage adverse drug events and medication-related problems
  • Provide medication management support for patients, including preoperative medication reviews and postoperative follow-up as appropriate
  • Assist with anticoagulation management for patients including perioperative holding, bridging, and resumption of therapy
  • Provide education to patients, caregivers, and staff regarding medication use, safety, and best practices
  • Support formulary management and standardization of medication protocols and order sets
  • Participate in medication safety initiatives including event review and risk-reduction strategies
  • Support compliance with regulatory and accreditation standards (Joint Commission, CMS, state board)
  • Assist in development and maintenance of medication-related policies and clinical guidelines
  • Serve as a clinical resource to physicians, APNs, nursing staff, and other healthcare professionals
  • Participate in interdisciplinary rounds, care conferences, and performance improvement initiatives
  • Support transitions of care by facilitating accurate medication reconciliation
  • Participate in training and mentorship of pharmacy staff, residents, students, or learners
  • Maintain professional competence through continuing education and adherence to best practice standards
  • Other duties as assigned

SCHEDULE:

PRN coverage of FT staff PTO.

Shift coverage includes 5:30am-1:30pm, 8:00am-4:00pm, or 10:00am-6:00pm. All Monday-Friday.

Ability to provide on-call coverage for nights, weekends, and holidays.

QUALIFICATIONS AND EDUCATION:

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Consult with medical staff regarding potential drug interactions, patient medical history, and use of particular medications.
  • Collaborate with medical staff to plan, monitor, review, and evaluate the quality safety and effectiveness of medication regimens.
  • Analyze prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions.
  • Assess the identity, strength, or purity of medications. Compound and dispense medications as prescribed by physicians and other providers by calculating, weighing, measuring, and mixing ingredients – or overseeing these activities.
  • Ensure pharmacy data is recorded, maintained, and comply with regulations, security, and OSS Health procedures.
  • Provide direct clinical pharmacy services to patients, including medication therapy review, optimization, and monitoring.
  • Participate in medication management, including antimicrobial prophylaxis, venous thromboembolism (VTE) prophylaxis, analgesia, and anesthesia‑related pharmacotherapy.
  • Support evidence‑based pain management strategies, including multimodal analgesia, opioid stewardship, and management of acute and chronic pain.
  • Review, verify, and optimize medication orders for appropriateness, safety, dosing, and therapeutic efficacy.
  • Identify, prevent, and manage adverse drug events and medication‑related problems, including allergies, drug interactions, and contraindications.
  • Provide medication management support for patients, including preoperative medication reviews and postoperative follow‑up as appropriate.
  • Assist with anticoagulation management for patients including perioperative holding, bridging, and resumption of therapy.
  • Provide education to patients, caregivers, and staff regarding medication use, safety, and best practices.

  • Serve as a clinical resource to physicians, advanced practice providers, nursing staff, and other healthcare professionals.
  • Participate in interdisciplinary rounds, care conferences, and performance improvement initiatives as appropriate.
  • Support transitions of care by facilitating accurate medication reconciliation at admission, transfer, and discharge.
  • Participate in training and mentorship of pharmacy staff, residents, students, or learners as applicable.
  • Maintain professional competence through continuing education and adherence to best practice standards.
  • Other duties as assigned.
