Job summary

OSS Health in Pennsylvania is seeking a PRN pharmacist to provide comprehensive pharmacy services across inpatient, outpatient, and surgical settings. The role includes medication reviews, safety monitoring, and collaboration with medical staff to optimize regimens.

Responsibilities

  • Consult with medical staff on drug interactions and patient history
  • Collaborate to plan, monitor, and evaluate medication regimens
  • Analyze prescribing trends to ensure patient compliance and prevent adverse interactions
  • Assess identity, strength, and purity of medications and dispense as prescribed
  • Ensure data is recorded and compliant with OSS Health procedures
  • Provide direct clinical pharmacy services and medication management
  • Participate in antimicrobial prophylaxis, VTE prophylaxis, analgesia, and anesthesia-related pharmacotherapy
  • Support multimodal analgesia and opioid stewardship
  • Review and optimize medication orders for safety and efficacy
  • Identify and manage adverse drug events and drug interactions
  • Provide patient education on medication use and safety
  • Support formulary management and standardization of protocols
  • Contribute to medication safety initiatives and regulatory compliance
  • Assist in transitions of care and medication reconciliation
  • Mentor pharmacy staff and learners
  • Maintain professional competence through continuing education
  • Other duties as assigned

Job description

SCHEDULE:

PRN coverage of FT staff PTO.

Shift coverage includes 5:30am-1:30pm, 8:00am-4:00pm, or 10:00am-6:00pm. All Monday-Friday.

Ability to provide on‑call coverage for nights, weekends, and holidays.

QUALIFICATIONS AND EDUCATION:
  • Doctor of Pharmacy (PharmD) or Registered Pharmacist license from an accredited college or school of pharmacy
  • Active pharmacist license in the State of Pennsylvania
  • Completion of a PGY1 pharmacy residency or equivalent clinical experience
  • Strong knowledge of pharmacotherapy principles, medication safety, and clinical practice standards
  • Ability to work effectively in inpatient, outpatient, and surgical care environments
  • Experience with electronic health records and clinical decision support systems
  • Excellent interpersonal and customer service skills
  • Strong attention to detail, organizational skills, and ability to solve problems
  • Ability to balance competing priorities
  • Commitment to patient‑centered care and continuous quality improvement
  • Knowledge of regulatory and accreditation requirements related to medication use
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Consult with medical staff regarding potential drug interactions, patient medical history, and use of particular medications.
  • Collaborate with medical staff to plan, monitor, review, and evaluate the quality safety and effectiveness of medication regimens.
  • Analyze prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions.
  • Assess the identity, strength, or purity of medications. Compound and dispense medications as prescribed by physicians and other providers by calculating, weighing, measuring, and mixing ingredients – or overseeing these activities.
  • Ensure pharmacy data is recorded, maintained, and comply with regulations, security, and OSS Health procedures.
  • Provide direct clinical pharmacy services to patients, including medication therapy review, optimization, and monitoring.
  • Participate in medication management, including antimicrobial prophylaxis, venous thromboembolism (VTE) prophylaxis, analgesia, and anesthesia‑related pharmacotherapy.
  • Support evidence‑based pain management strategies, including multimodal analgesia, opioid stewardship, and management of acute and chronic pain.
  • Review, verify, and optimize medication orders for appropriateness, safety, dosing, and therapeutic efficacy.
  • Identify, prevent, and manage adverse drug events and medication‑related problems, including allergies, drug interactions, and contraindications.
  • Provide medication management support for patients, including preoperative medication reviews and postoperative follow‑up as appropriate.
  • Assist with anticoagulation management for patients including perioperative holding, bridging, and resumption of therapy.
  • Provide education to patients, caregivers, and staff regarding medication use, safety, and best practices.
  • Support formulary management and standardization of medication protocols and order sets.
  • Participate in medication safety initiatives, including contributing to medication safety programs, including event review, root cause analysis, and implementation of risk‑reduction strategies.
  • Support compliance with regulatory and accreditation standards (e.g., Joint Commission, CMS, state board of pharmacy).
  • Assist in development, review, and maintenance of medication‑related policies, procedures, and clinical guidelines relevant to orthopedic practice.
  • Serve as a clinical resource to physicians, advanced practice providers, nursing staff, and other healthcare professionals.
  • Participate in interdisciplinary rounds, care conferences, and performance improvement initiatives as appropriate.
  • Support transitions of care by facilitating accurate medication reconciliation at admission, transfer, and discharge.
  • Participate in training and mentorship of pharmacy staff, residents, students, or learners as applicable.
  • Maintain professional competence through continuing education and adherence to best practice standards.
  • Other duties as assigned.
