Qualifications

• Graduation from an accredited school of pharmacy with a five year Bachelor’s Degree in pharmacy or a six year entry level Pharmacy D. degree.
• Completion of pharmacy internship and licensure as a Registered Pharmacist by passing examination of the NY State Board of Pharmacy or by the reciprocal licensure.
• Participates in orientation and continuing education and updates and maintains knowledge and skills related to specific areas of expertise.

Responsibilities

• Provides pharmaceutical service and care to all patient populations based on individual variables and disease state.
• Maintains patient-specific aspects of care in dosing based on age, weight, and renal function.
• Prepares inpatient and outpatient prescription orders and refills medications.
