Job summary

Trinity Health Of New England is seeking a pharmacist to provide comprehensive pharmacy care across acute and ambulatory care settings, including clinical, retail, and specialty services. You will lead quality initiatives, oversee medication order review, and mentor staff within Saint Francis Hospital’s pharmacy program.

The role requires a Pharm.D. or Bachelor’s in Pharmacy with 3 years of pharmacist experience, CT license, and dedication to evidence-based practice and patient safety.

Qualifications

• Bachelor’s Degree in Pharmacy, Doctor of Pharmacy (Pharm.D.) preferred.
• 3 years’ experience as a pharmacist, residency preferred; internal or external certifications; Board of Pharmacy Specialists certification preferred.
• CT State Pharmacist License

Responsibilities

• Lead quality and compliance programs and provide subject matter expertise (e.g., compliance, compounding, diversion, quality)
• Design and implement pharmacy-based and regional performance improvement initiatives to ensure safe use of medications
• Provide oversight and guidance for medication order review, preparation, distribution, and dispensing processes
• Monitor and evaluate drug therapies to promote optimal drug use and evidence-based practices to reduce risks of drug misadventures
• Lead medication safety measures and efficient operational processes
• Provide drug information and therapeutic recommendations based on patient specific parameters and conditions
• Perform medication therapy management (MTM) and comprehensive medication reviews (CMR)
• Lead training, development, mentoring and assessment of competencies (e.g., Residency Preceptor)
