Advocate Trinity Hospital - Pharmacy is seeking a part-time pharmacist to oversee pharmaceutical care, counsel patients and caregivers on medications, and ensure safe handling and dispensing. The position requires pharmacy licensure in the state, immunization certification, and a degree from an ACPE-accredited school.

Qualifications

• Licensed in the state of practice or eligible for reciprocity
• Immunization and non-vaccine injectable certification required
• Pharmacy degree from ACPE-accredited school or NABP FPGEC certificate

Responsibilities

• Oversees pharmaceutical care, evaluating biomedical literature and designing therapy regimens
• Provides patient and caregiver counseling on medication therapy, adverse effects, compliance, and administration
• Designs and monitors patient-specific regimens using biomedical literature and policies
• Interprets medication orders and prescriptions accurately
• Participates in medical emergencies and supports medication safety and quality improvement
• Collaborates with the healthcare team to manage formularies and medication-use processes
• Prepares, dispenses, and ensures proper storage and handling of medications
• Serves as a preceptor and mentors pharmacy teammates, residents, and students
