St. Joseph's Health in Jersey City, NJ, seeks a Pharmacist to perform and document clinical interventions, education, and counseling related to medication safety and management across inpatient and outpatient settings.

You will interpret orders, prepare and dispense medications, assess interactions, allergies, duplications, and dosing to ensure the right patient receives the right therapy at the right time. NJ licensure and ACLS within one year are required or preferred.

Formación

• PharmD required; BS Pharmacy may be accepted as entry level.
• NJ Board of Pharmacy licensure and registration required.
• Pharmacy Residency and BCPS board certification preferred.
• New hires must obtain ACLS within one year.
• Direct patient care to neonate through geriatric patients.
• Annual CEUs: 25 CEUs approved by Pharmacy Management.

Responsabilidades

• Perform and document patient pharmacy clinical interventions, education, and counseling.
• Interpret, evaluate and review physician orders and outpatient prescriptions for accuracy and safety.
• Assess drug interactions, allergies, duplications, dosing, and therapeutic indications.
• Collaborate with healthcare team to ensure optimal therapy and patient safety.
