Job summary

UMass Memorial Health is seeking a pharmacist to participate in drug therapy management and interdisciplinary reviews. You will prepare, dispense and optimize medications, while coordinating with physicians to ensure effective therapy and patient safety.

Responsibilities include patient access assistance, documentation, and adherence to state/federal pharmacy laws. This role requires MA licensure, PharmD or BSPharm, and ambulatory experience in a hospital setting.

Qualifications

• Pharm.D or BSPharm degree required.
• Licensed in the Commonwealth of Massachusetts.
• 3 years ambulatory experience required.
• Must have essential motivational interviewing skills to engage patients in their health care.

Responsibilities

• Participates in drug therapy management and documents care in medical records.
• Interdisciplinary reviews of patient progress and communicates changes to other health care providers.
• Prepares medications by reviewing physician orders and detecting incompatibilities.
• Dispenses medications by compounding, packaging, and labeling pharmaceuticals.
• Contacts and consults with physicians to optimize drug therapy and advise on interventions.
• Assists patients in gaining access to medications and resolves payer issues when applicable.
