• Participates in drug therapy management and documents care in patients' records.
• Interdisciplinary reviews of patients' progress and communicates with other providers.
• Prepares medications by reviewing orders, detecting incompatibilities, and ensuring accuracy.
• Dispenses medications by compounding, packaging, and labeling.
• Collaborates with physicians to optimize drug therapy and advise on interventions.
• Assist patients in gaining access to medications, including payer interactions.
• Provide pharmacological information to health care professionals and patients.
