Job summary

UMass Memorial Health is seeking a clinical pharmacist to provide comprehensive medication management and patient education. This role supports clinical services, order management, and research, reporting to an immediate supervisor.

Location is in a hospital setting with an evening shift. Responsibilities include reviewing med histories, developing therapeutic plans, monitoring outcomes, and ensuring safe, effective medication use.

Qualifications

• Pharm.D degree or BSPharm with 3 years hospital experience.
• Massachusetts Board of Registration in Pharmacy licensure.
• ASHP Accredited Residency (PGY1) recommended.
• Evening Shift 2p-10:30p.

Responsibilities

• Review patient medication history and educate patients on drug therapy.
• Develop and modify therapeutic plans and determine alternatives to achieve outcomes.
• Monitor drug therapy for appropriateness, interactions, and adverse events.
• Facilitate medication discharge planning and transitions to other care sites.
• Supervise pharmacy technicians and ensure regulatory compliance.
