Job summary

UMass Memorial Health in Worcester, MA is seeking a Clinical Pharmacist to provide comprehensive medication management, education, and clinical services within a hospital setting. The role focuses on patient safety, outcomes, and collaboration with care teams in a busy pharmacy environment.

The pharmacist will participate in education of IPPE and APPE students and residents, conduct drug information, order reviews, and support formulary and cost-saving initiatives, with weekend and holiday

Qualifications

• Pharm.D degree required; or BSPharm with 3 years hospital experience.
• Licensure with the Massachusetts Board of Registration in Pharmacy is required.
• ASHP Accredited Residency in Pharmacy Practice (PGY1) is recommended.
• Pharm.D with minimum of 1 year hospital experience preferred.

Responsibilities

• Provides comprehensive medication management services in a collaborative manner.
• Maintains patient profiles and monitors drug therapy for appropriateness and interactions.
• Precepts/IPPE/APPE students and pharmacy residents as applicable.
• Supervises pharmacy technicians and participates in regulatory compliance.
• Contributes to formulary management and cost savings initiatives.
• Performs medication order review and counseling at discharge.
