Job summary

Advocate Health is seeking a licensed pharmacist to lead clinical pharmacy activities in Wisconsin. You will oversee pharmaceutical care, review orders, and design individualized regimens based on biomedical literature.

Counseling patients and caregivers on therapy, adverse effects, and proper administration is essential. The role includes participating in emergencies, supporting medication safety initiatives, and mentoring pharmacy staff and students.

Qualifications

• Requires pharmacy licensure in current practice state
• PharmD or Doctorate in Pharmacy from ACPE-accredited school, or NABP FPGEC certificate
• Requires ability to counsel patients and educate caregivers effectively

Responsibilities

• Oversees pharmaceutical care, retrieving and evaluating biomedical literature and designing medication therapy regimens
• Provides patient and caregiver counseling on medication therapy, adverse effects, compliance, handling, and administration
• Designs, monitors and evaluates patient-specific pharmacotherapeutic regimens based on literature
• Reviews and interprets medication orders and prescriptions accurately
• Participates in management of medical emergencies when applicable
• Supports medication safety and quality improvement activities and reports events
• Collaborates on formulary adherence and medication-use process; assists with non-formulary/investigational meds
• Ensures compliance with policies, procedures, and regulations; proper storage and handling of medications
• Serves as preceptor for students and residents; mentors and trains pharmacy staff
• Directs and checks accuracy of work of pharmacy support team to ensure safety and efficiency
• Demonstrates age-appropriate patient care and life-span knowledge as required by department standards
• Must perform PPE use and safety measures to minimize exposure to hazardous agents
• Must be able to perform writing, typing, and data entry and operate dispensing machines
