Qualifications

• Bachelor or PharmD from an accredited institution.
• Current Ohio pharmacist license.
• BLS certification within orientation.
• ACLS certification may be required by manager.
• Pharmacokinetic dosing experience.
• Drug therapy management experience.
• Collaborative practice experience with providers.
• ASHP residency or equivalent experience.
• Pharmacy board certifications or other certifications.
• Advanced degree in healthcare field.

Responsabilités

• Review provider orders for correct drug choice, dosage form, dosing schedule, and interactions.
• Dispense and compound medications following OhioHealth policies and ensure proper labeling.
• Counsel patients on medications and health topics and provide drug information as needed.
• Interpret prescriptions and manage patient drug therapy in collaboration with providers.
• Precept pharmacy students and residents and provide educational programs.
• Direct and guide pharmacy technicians and support personnel.
• Participate in medication safety, stewardship, and utilization initiatives.
