Job summary

Reedsburg Area Medical Center in Wisconsin is seeking a Clinical Pharmacist to provide medication therapy management and collaborate with providers to ensure safe, effective, and cost-efficient drug therapy. The role supports rounds, reconciliation at admission/discharge, therapy monitoring, education, and drug information.

The Pharmacist will train residents and students, operate within department policies, and serve hospital, clinics, nursing homes, and outpatient teams, including handling

Qualifications

• Pharmacy degree from an accredited college of pharmacy.
• ASHP accredited PGY-1 residency preferred.
• LTC consultation experience preferred.
• Wisconsin licensure to practice required.
• Board certification preferred.

Responsibilities

• Provide medication therapy management in collaboration with providers.
• Participate in patient care rounds, reconciliation, and therapy monitoring.
• Manage medication orders and provide drug information.
• Educate patients and contribute to clinical training of residents and students.
• Handle hazardous drugs with appropriate safety controls.
