Job summary

Weston County Health Services is seeking a PRN Pharmacist for our Hospital Pharmacy. The pharmacist will provide medication therapy management in collaboration with other providers to ensure safe, effective, efficient, and cost-conscious care.

Responsibilities include medication reconciliation, patient counseling, drug information, dosing, and education activities within established policies and guidelines. Qualifications include Doctorate in Pharmacy and Basic Life Support certification.

Qualifications

• Pharmacist must hold a Doctorate in Pharmacy and be licensed or eligible.
• BLS certification is required or current.

Responsibilities

• Maintains knowledge of state and federal pharmacy laws and CMS rules for relevant facilities.
• Performs medication management, reconciliation, and patient education.
• Supports medication safety, adverse event reporting, and inventory control.
• Assists with 340b program and billing as needed.
• Serves as a resource to medical and nursing staff to ensure safe, effective therapy.
