Job summary

UW Health in Madison, WI, is seeking a Clinical Pharmacist to join our pharmacy team. The 100% FTE role includes 70% AM shifts (7:00 AM - 3:30 PM) and 30% PM shifts (1:30 PM - 10:00 PM) with a 1-in-4 weekends/holidays rotation.

Qualifications

• Bachelor's Degree in Pharmacy or PharmD required.
• Completion of ASHP Accredited Residency preferred.
• 1 year of clinical patient care and orders management experience required.
• 3 years of institutional experience or ASHP residency preferred.
• Wisconsin pharmacy license within 90 days required.
• BLS within 90 days required; ACLS within 180 days required.

Responsibilities

• Provide medication therapy management as part of interdisciplinary team for inpatient services.
• Perform clinical and distributive tasks to ensure safe, effective and timely medication therapy.
• Provide training and education to Pharmacy Residents and PharmD students.
• Provide patient care to a diverse inpatient population.
