Qualifikationen

• PharmD or PharmD-equivalent degree required.
• Current Pharmacist license in the State of Ohio.
• Prior Pharmacist experience required, with closed-door/production pharmacy preferred.

Aufgaben

• Ensure clinical accuracy and execution of patient prescriptions.
• Verify hard copy and digital Rx's.
• Communicate with healthcare professionals and ensure the Dr call line is answered.
• Supervise and execute incoming and outgoing prescription transfers.
• Perform DUR to identify MTM eligible patients and identify clinical interventions with follow-up.
• Ensure all legal, privacy and compliance requirements are met.
