Qualifications

• Experience not required but knowledge in clinical pharmacy is expected.

Responsibilities

• Oversees pharmaceutical care, including reviewing biomedical literature and designing therapy regimens.
• Provides patient counseling on medication therapy, adverse effects, adherence, and proper use.
• Designs and monitors patient-specific regimens and documents changes per policies.
• Reviews medication orders for accuracy and safety; participates in emergencies when needed.
• Supports medication safety and quality improvement across the department.
