Job summary

PharMerica in Leesport, PA is seeking a licensed Pharmacist for a full-time, non-retail role serving long-term care and senior living clients. You’ll review orders, optimize therapy, monitor high-risk meds, and collaborate with physicians and nurses in a closed-door setting.

Qualifications

• Experience with medication therapy management and drug interactions.
• Knowledge of CMS/DEA/FDA standards.
• Strong written and verbal communication.
• Proficient in documenting interventions in patient records.

Responsibilities

• Evaluate physician medication orders for drug appropriateness and dosage.
• Provide clinical recommendations on medication therapy changes.
• Maintain up-to-date patient profiles with meds, labs, history.
• Order/monitor lab tests for high-risk meds as needed.
• Collaborate with physicians, nurses, and health staff on therapy.
• Document communications and interventions in medical records.
• Enforce policies for safe handling and dispensing of meds.
• Ensure compliance with state/federal regulatory requirements.
• Report medication errors and adverse events to Pharmacy Manager.
• Educate clinical team on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
