The role emphasizes collaboration with physicians, nurses, and staff, with opportunities for ongoing training and advancement in a growing company. Prior LTC experience is preferred.

Qualifications

• Licensed pharmacist in the state of practice.
• Experience in long-term care preferred.
• Knowledge of CMS/DEA/FDA guidelines.
• Proficient with EMRs and pharmacy systems.

Responsibilities

• Evaluate physician orders for drug appropriateness and dosing.
• Provide clinical recommendations to physicians and staff.
• Maintain resident profiles with meds and labs.
• Ensure labs are ordered/reviewed for high-risk meds.
• Collaborate with clinicians to support therapy changes.
• Document interventions in patient medical records.
• Ensure compliance with regulatory requirements.
• Educate clinical team on safe medication handling.
