Job summary

PharMerica, a BrightSpring Health Services affiliate, is hiring a full-time Pharmacist in Leesport, PA. You’ll work in a non-retail, closed-door setting serving long-term care and senior living clients.

Responsibilities include evaluating orders, making clinical recommendations, maintaining patient profiles, and ensuring regulatory compliance. Benefits include health, dental, vision, and more. Must hold an active pharmacist license in the state and have LTC experience.

Qualifications

• Active pharmacist license in the state of practice.
• At least 1 year of long-term care experience.
• Strong clinical judgement and patient safety focus.
• Excellent written and verbal communication skills.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and route of administration.
• Provide clinical recommendations to physicians and staff on therapy changes.
• Maintain resident profiles with medical history, labs and meds.
• Ensure monitoring labs for high-risk meds are ordered, reviewed, and communicated to the team.
• Collaborate with physicians and nurses to provide clinical and technical support.
• Document communications and interventions in patient records, including families and caregivers.
