Job summary

PharMerica in Bensenville, IL is seeking a part-time Pharmacist to join a closed-door, non-retail team serving long-term care and senior living clients.

You will review orders, provide clinical recommendations, maintain patient profiles, ensure proper drug therapy, and collaborate with physicians and nurses. The role offers weekday scheduling, ongoing training, advancement opportunities, and comprehensive benefits.

Qualifications

• Bachelor’s Degree in Pharmacy; Pharm D preferred.
• One year experience in long term care desired.
• Current, valid pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards.
• Proficiency in electronic medical records and pharmacy information systems.
• Strong clinical judgement and effective written and verbal communication.
• Position will be posted for a minimum of 7 business days.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
• Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
• Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications.
• Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.
• Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
• Document all communications and clinical interventions in the patient’s medical record.
• Implement and enforce policies for safe handling, dispensing, and clinical oversight of medications.
• Ensure compliance with state and federal regulatory requirements related to pharmacy practice and medication management.
• Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
• Educate and support clinical team and healthcare agency staff on safe medication handling procedures.
• Perform other pharmacy-related tasks and responsibilities as assigned.
